TARGETED CASE MANAGEMENT

Our mental health targeted case managers work in the community to seek affordable housing for our clients in pressing circumstances. We have partnered with various insurance providers who have adopted an housing assistance program called the housing pilot. Based on the client's circumstance and criteria, the client may be able to gain access to financial assistance to their housing crisis.
